home care insurance

Medicade only pays for the first month, right? I know that a person’s personal resources pay for the home. What about Long term care insurance.

I have done a thousand searches for this answer but found nothing. Please advise me if you can.
I have a great job that I am good at, but I have some serious mental health problems. I am not a direct danger to myself or anyone else, I am not violent or anything. The problem is that I don’t eat, don’t take care of myself, stop taking meds for no reason, have episodes where I am unable to leave the house- and it gets worse from there. But I seem to be able to pull it together for work. I should not live alone, but I have nobody to look after me.
I am only 37, I have a good income, and I am able to drive myself. I don’t need a lock-down inpatient unit.
I live in Phoenix, AZ.
In a perfect world I would live in what I would describe as a nursing home for young people like me that need help but don’t need to be locked up.
What options do I have? Any ideas? I am very scared that I am going to lose it and fall apart, die, get sick, hurt, or I don’t know what. I am not well enough to take care of myself, but I am self aware enough that I know I need someone to check in on me to make sure I am ok. I have no friends and no family in the state (not that I would want to burden them with this anyway).
Are there services that I could get to have someone stop by my house?
Any suggestions would be appreciated; I am at the end of my rope here.
Thank you,
Ted
Phoenix, AZ
